Approved Vet List

It is required for you to take your folder and any records you have to every vet appointment.

When you get a new foster, the following things will occur:

  1. We will mail you a folder containing a flea/tick preventative, an ID tag, and a microchip, if necessary.

    • You can pick this folder up at our headquarters if you live in the Whitewater area or will need to come for other supplies.​

  2. You will receive an email from notifications@cognitoforms.com. The ADL Checklist will include the following:

    • Dog’s name

    • Dog’s estimated age

    • Your name and email address

    • What vaccines have been administered and what vaccines are still required

    • If the dog has a chip or one is still required

    • If the dog requires a 4DX test (tests for heartworm and tick-borne diseases)

    • Date last heartworm preventative was administered and/or date it is due

    • Date last flea/tick preventative was administered and/or date it is due

    • CVI expiration date (only valid 30 days and a new one will need to be issued at the time of adoption if no longer valid)

    • Any additional comments

    • Who completed the form and the date it was completed

  • It is recommended that you print out the ADL Checklist and add it to your folder once you receive it.
